Purpose is to provide a diagnosis of present psychiatric disorders (ADHD, Bipolar, etc.) and treatment. This may include lab orders or the prescribing of medication. Substance Abuse treatment cost an additional $10 for required drug screening.

Purpose is to provide an ASAM assessment consisting of psychiatric diagnosis, substance use risk assessment, readiness for treatment and appropriate treatment options. This may include the ordering of labs and an in office urine drug screen.

Purpose is to provide diagnostic information about what medications/substances are being taken and what medications/substances are not being taken.
We do not accept insurance. However, we are happy to provide you with an itemized statement and directions for you to file out-of-network with your private insurance company. For insurance reimbursement, you must pay the flat rate for them to accept it. Not accepting insurance allows us to keep costs at a more affordable price for those who are under insured or uninsured. Please verify with your insurance that they reimburse for out-of-network providers.
